Our new name is Health Equity Rising
We are happy to announce our new organization name: Health Equity Rising! In May 2025, Sonoma Connect and Health Action Together merged as one organization with a commitment to strengthen how Sonoma County’s health and social service systems work with the people we serve, embed equity, and transform into healing-centered systems. This merger has impulsed our work and to communicate our collective vision we have decided to change our name and branding to something that represents who we are as one entity.
After undergoing a rebranding process with community engagement where we elicited feedback from 48 individuals, including members of our board, staff, cross-sector partners, and the community, we have chosen our new name and logo. Health Equity Rising represents the mission of our organization, which is to co-create a healing-centered ecosystem with communities facing systemic harm by:
uplifting and centering community leadership and priorities,
building a culturally responsive infrastructure through the Community Information Exchange Model
We chose a representation of a dandelion as our logo because it symbolizes our vision of having a healing-centered community where everyone is connected, flourishing, and liberated from harm. With a resilient spirit, dandelions disperse their seeds by releasing them in the air where they travel with the wind and can spread up to 500 miles away. Like dandelion seeds, we hope that frameworks and infrastructures that center community voice, connection, and equity will continue to spread throughout Sonoma County and beyond, transforming our communities into places where people can flourish.
Following this announcement, we will continue to share the news with more partners in meetings, events, and communications. We will be developing a new website with updated language and information and will have more resources to share with you soon!
