Please consider supporting Health Action Together. Together we support change efforts in the Sonoma County neighborhoods and communities most historically excluded from decision making. These efforts are led by individuals who have developed the Agenda for Action, a blueprint for tackling systemic inequities that have disproportionately affected the county's underserved neighborhoods.
