We are excited to announce that Health Action Together has merged with Sonoma Connect!

We are a collaborative of community leaders, nonprofits, and multi-sector partners centering the needs of people most impacted by structural racism to lead change efforts to transform Sonoma County, through community-led action and information sharing, into a place where every person can thrive. 

Why did we decide to merge?

  • We underwent deliberation with both of our Community Leadership Boards and identified that our efforts both center people on the margins and embed equity 

  • We both approach work through a collaborative lens as conveners of multi-sector partners, community leaders, and nonprofit organizations. Together we are stronger, work to advance common goals, and have a wider network of partners to engage 

  • Uniting our teams means less duplication of work and less competition for funding and resources. We value collaboration across sectors and organizations and believe that a healing-centered system prioritizes the wellbeing of our community through shared resources 

  • Together we can both advance goals identified by community members in the Agenda for Action. Sonoma Connect | Sonoma Unidos leads the development of a local Community Information Exchange Model and uses the integrated technology, the Resource Connection Network to promote closed-loop referrals across organizations and sectors to better support community members navigate and access resources. The Agenda for Action calls for increased access to resources and the Community Information Exchange model works to reduce barriers that people experience when attempting to access resources and supports in Sonoma County, directly supporting priority areas in the Agenda for Action

  • We believe in the potential and wellbeing of our staff and community leaders and that together we can cultivate a welcoming, healing-centered space of belonging in our community 

Sonoma Connect | Sonoma Unidos has been undergoing a Strategic Planning Process with Equity First Consulting that includes the work of Health Action Together and we will be sharing more about this process soon and opening it to our wider network for input. Using this process as guidance, we will be engaging YOU to help us in the next steps of our merged branding. We will be combining our websites, newsletters, and social media accounts and will be sharing more information about our work together.
