Health Action Together is a non-profit public-private partnership leading and supporting cross-sector efforts to reduce health, education, and income inequities across Sonoma County.

Health Action Together aims to address inequities in our community by centering the communities most impacted to drive policy, institutional, and resource and investment changes. By centering community, we will facilitate systems change efforts that lead to shared decision-making and power among populations impacted by inequities and local governments, nonprofits, philanthropy, and businesses.
